Community Health Needs Assessment (CHNA)
As part of the Affordable Care Act of 2010, freestanding non-profit hospitals are required to conduct a Community Health Needs Assessment (CHNA) every three years. The Community Health Needs Assessment (CHNA) describes the health status of the hospital’s community, and determines the overall health needs of the local populous.
In response to the increased needs and standards in our community, we have analyzed market data, demographic data, socio-economic factors, and health service utilization rates. We also received counsel from community coalitions, boards, partnerships, advisory groups and panels in order to bring you what we think is the best community health needs assessment for our area.
Unmet community health needs have been identified in the following areas:
- Expansions of primary care and specialty services
- Increased collaboration with mental health providers
- An increase in cooperation, communication, and education across communities for major health issues
- Local and regional approaches to emergency medical services
In addition, there are three overarching themes that should be addressed in each priority area:
- Physical healthcare needs
- Behavior healthcare needs
- Re-integration challenges

Heart of Texas Healthcare System has always believed in assisting our patients in making informed decisions for their healthcare journey.
New guidelines established by the United States Department of Health and Human Services require us to publish our negotiated rates with payers for services provided at our facility. We understand the cost of healthcare can be very complex and confusing. Price transparency is important and that is why we are helping you to understand the complexities of your insurance coverage and your out-of-pocket costs while maintaining the quality of the services we are providing to you.
While we have made these tools available to you, we encourage you to have one-on-one conversations with your care team or admitting representative with questions about costs for your medical needs.
